As a whānau-focused medical practice, we are committed to providing you and your whānau with a broad range of healthcare services.
Health365 provides secure access to your own medical records. You can also be authorised to view other family members’ records on the same account (e.g. children or dependents). You can order repeat prescriptions, access your health records, view and track your test results.
How do I get access to Health365?
Call us on 0800 573 0091 to sign up. Once you receive the email from us, follow the link to the website and enter the username and password provided.

Test Results
Our policy at Poutiri Wellness Centre is we do not routinely notify patients of normal test results unless otherwise specified by their doctor. We will only notify you of your results if they are of concern. If you would like to know your results or you were expecting a call from us, please phone the Clinic 0800 573 0091 and speak to the Practice Nurse
After Hours Service/ Emergencies
For all emergencies, please call 111 for immediate assistance
If you require medical assistance after hours, you can phone the Clinic 0800 573 0091. The phone will be answered by Whakarongorau Aotearoa (formerly Homecare Medical Services), you will be provided with medical advice and information by a Registered Nurse, and they will be able to help redirect you.
Or Practice Plus is our trusted virtual health provider, available weekdays until 10pm and weekends and public holidays 8am – 8pm. All Practice Plus Clinicians are experienced and licensed practitioners, registered with either Medical Council of New Zealand, or Nursing Council of New Zealand.
Through video or phone technology, a Practice Plus Clinician can privately and securely help treat most medical needs, sicknesses or injuries. From a common cold, contraceptive options, migraines and more, Practice Plus can provide treatment and prescriptions direct to your pharmacy along with health counselling and off-work certificates, as well as many more conditions.
Charges are $65 for ages 14 and over, $55 for ages 13 and under, or if you are a Community Service Card holder, you will be charged $19.50 for the appointment.
You will be asked to enter your credit or debit card details when booking your appointment, however your card will not be charged until your appointment is completed. For more information click here https://practiceplus.nz/
